The online 6007 Health and Consent form is now available! Click on your camp below for instructions and camp-specific requirements. Now that the new form has launched, any previous paper 6007 will not be considered valid. For summer 2026, if areas bring an old version of camp, parents will need to complete an updated form before their child can participate in any activities. Please stop using, distributing or saving any older versions of the form to avoid confusion.
