Summer 2025 Health and Consent Form Process

To identify who still needs to fill out a Health and Consent Form:

  1. In YL Connect Camp, on your arrival form, you can view the HC Form Status of each camper.
  2. If a registrant has an HC Form Status of “completed online”
  3. If a registrant has a status of “Awaiting Physician signature,” they either need to bring a paper copy of the signature to camp or use the 6007 Health and Consent form to upload a copy of the physician’s signature.
  4. If a registrant's HC Form Status is anything other than “completed online,” “completed paper,” or “Awaiting Physician Signature,” parents/ guardians must fill out the 6007 Health and Consent form

To communicate with parents and guardians who still need to utilize the Summer 2024 Health and Consent form:

  1. From the arrival form in YLConnect Camp, go to Send Individualized Email​ and select the HC Form Invite.
  2. You will then be able to select which registrants should receive the email.
  3. Click send!

Tracking Submissions and Preparing for Travel:

  1. As parents/guardians complete the form, you will receive emails from Cognito with PDFs of the completed forms. (Parents and camps will also have access to copies.)
  2. Please track and ensure all forms are completed. In YLConnect Camp, as forms are submitted, you should change a registrant's HC Status to “Completed in Cognito.” Please note that there is no automation between YLConnect Camp and the Cognito form. You must manually update the status to “Completed in Cognito.”
  3. Print the PDF forms for use during travel to and from camp.
  4. If a camper arrives without a completed health and consent form, the trip leader and camp team will work together to have parents/guardians complete a form as soon as possible.

Summer 2024 Information

  • YL Connect Camp​ can be used for registration, payments and trip management tools. A standard registration template has already been added to your YL Connect account and is ready to be customized for your trip.

Before Camp

Health and safety at camp starts prior to the trip. Please encourage all campers and leaders to be aware of any potential exposure leading up to camp. It’s imperative that every person coming to camp:

  • Is free of any COVID-19 symptoms and has a temperature under 100.4 prior to arriving at camp
  • The Event Medical Screening form​ must be completed for the entire group prior to leaving the local area.
  • Lunch is not provided on day 1.​ In planning lunch, please consider a picnic/tailgate lunch to avoid eating inside.

At Camp

  • Yurts or cabins are open air, meaning each bunk is adjacent to open window, which allows for ample airflow.
  • In the yurts, bunks are spaced so everyone can sleep head to toe.
  • Everyone will need bring a sleeping bag and towel(s), which limits the possibility of exposure.
  • Camp will provide each person with a hammock and Crazy Creek camp chair to use for the week. A select number of Nalgene water bottles are also available.
  • Campers will eat all meals outside.
  • All group gatherings will also take place outside under our open-air shelter or around the campfire.
  • Handwashing stations with biodegradable soap will be available at the water tank and in the kitchen.
  • Hand sanitizer is available on every table, in each yurt, and at each bathroom.
  • Our staff will sanitize bathrooms, high traffic/touch areas, and shared equipment daily.
  • On a full week, AWR does not have enough housing to isolate anyone. Upon showing symptoms, the symptomatic guest will need to arrange immediate pickup.

On Adventures

  • Campers will ride to adventures in their yurt groups.
  • Adventures include rafting, kayaking, hiking, and rock climbing.
  • Camp will provide tandem kayaks. Kayaking partners need to be from the same yurt group.
  • For rafting, a bus will transport campers to and from the river.

Required Documents

  • YL Connect Camp is required for summer 2024. The YLCC Arrival Form replaces the registration packet/A form excel document. Training materials​ on YL Connect Camp are available through Learning Guides as well as a Quick Guide with videos. The Arrival Form specific information can be found here.

Additional Resources

  • Arrival on Day 1 is at 3 PM. Departure on Day 5 is on or before 11 AM and before lunch.
  • Please make sure all of your vehicles are full of gas since you will need to drive your vehicles to and from adventures.
