Packing List

Colorado Camping Specifics

  • Physician Signature Information: Please click HERE to see all information for parents in regards to physician signature requirements for the state of Colorado or keep reading below. Please note that all WyldLife weeks will require physician signatures for all campers.
  • Medication Collection: Due to our childcare license in the State of Colorado, we are required to collect ALL medications, even over-the-counter meds. Please read through all of our Medication Collection Requirements. These meds will be collected as soon as you arrive to camp.
  • Immunization Records: Due to our childcare license in the State of Colorado, we are required to have immunization records on hand for all campers. When checking through online health and consent forms, make sure that the camper's have an uploaded immunization record online or that you receive a hard copy. If a parent needs to fill in an immunization record, they can use this Colorado Immunization Form.
  • CO Background Check & Fingerprints: At least 1 leader from every area will be required to be fingerprinted through Identogo in order to come to camp. These background checks for leaders are good for five years. More information will come about these background checks in April after R2.
  • Leader Requirements: Make sure you have a YL Volunteer Application on file for every leader, that they have been background checked, and are current in Staff Resources. Not only is this required by Young Life to operate your area, Trip Leaders will also be required to sign off on the following affidavit on their 10-Day Call in form: “I certify under penalty of perjury that the volunteer leaders and staff listed on the left are included in YL’s Staff and Volunteer Roster, and as such, each of those leaders have submitted an application, obtained three or more references, submitted to background checks as required by YL, reviewed and executed Young Life’s Statement of Faith, and have received training on child protection and safety as well as training on required mandatory reporting situations. I further certify that I have their YL Applications and related documents on file in our local Area."

Health Form Specifics

A sufficiently complete YL Health form is required at Crooked Creek Ranch for all campers and leaders. For summer 2024 Crooked Creek Ranch will be operating under a waiver from the State of Colorado on the Physician Signature regulation. Please carefully read through the below information on how to proceed with campers' health forms. The guides below will help you know what campers require a physician signature and which do not. Anyone over 18 will not require a physician's signature.

The waiver will apply to most campers. Here are the conditions that would make your campers ineligible for the waiver, thus requiring a Physician Signature prior to attending camp:

  • Has not completed the 9th grade by the start of the camp trip.
  • Has not provided complete parent and emergency contact information (this is standard health form information).
  • Has any diagnosed developmental or medical condition of the blood, neurologic, heart, respiratory, or metabolic system, including, but not limited to: seizures, diabetes, sickle cell disease, heart or respiratory conditions, and physical disabilities.
  • Has any diagnosed medical condition which requires the camper to carry an inhaler, EpiPen, or other emergency medication at all times.
  • Has asthma.
  • Has any severe allergy that requires medical intervention after exposure.
  • Is pregnant or gave birth less than 12 weeks before the first date of camp.

All WyldLife campers will require a physician signature.

Health Form (YL-6007)

Please ensure every camper and leader has completed a health form prior to your arrival to camp. For more details in regards to physician signatures, please look at the above section on Camping in Colorado Requirements.

It is helpful to make sure that you send an "individualized email" out of YL Connect which generates a registrant ID and access code and invites parents to complete the online health and consent form. The health and consent invitation is not automatically sent. When parents use the registrant ID and access code, health and consent information is synced to the correct camper and the correct summer camper camp trip in YL Connect.
